    McKesson Accelerates CommonWell Health Alliance® Efforts to Deliver Coordinated Care

    As a founding member of the CommonWell Health Alliance®, McKesson is dedicated to making patient data available to patients and providers – regardless of where care occurs. The goal is to put health information in the hands of caregivers and patients by providing access to health information technology systems, patient portals and apps. This can help providers increase efficiency, decrease costs and improve patient satisfaction.

    As part of its overall interoperability strategy, McKesson is enabling a suite of products across the care continuum to help providers connect across every care setting and successfully transition to value-based care.

    In April, the McKesson Homecare and McKesson Hospice products were first to market in enabling post-acute care in the CommonWell network. Product deployment is in the pilot stage, with several sites set to begin implementation later this year.

"As risk continues to shift to providers, hospitals will rely more on post-acute providers to help gain the efficiency of providing positive outcomes in a lower cost care setting."

"We can help improve both quality and efficiency of patient care by connecting providers in different care settings on disparate systems."

    The CommonWell Health Alliance has almost 5,000 provider sites enrolled; that number is expected to grow exponentially by the end of 2016.

    CommonWell has 49 members from across the care continuum with market leaders in acute and ambulatory care, long-term care, imaging, lab, retail pharmacy and more. Members comprise 72 percent of the acute EHR market and 37 percent of the acute EHR market. CommonWell is live in all 50 states, plus Puerto Rico and Washington, D.C.

    CommonWell services include:

    • Patient identification and linking: Assists health IT suppliers to quickly and accurately identify patients as they transition through care facilities.
    • Record locator: Helps providers locate and access their patients' records, regardless of where the encounter occurred, by providing a virtual table of contents that documents available data from each encounter location.
    • Patient access, privacy and consent management: Delivers a patient-authorized means to help simplify management of data sharing consents and authorizations.
    • Trusted data access: Provides certification, authentication and auditing services that facilitate consistent trusted data sharing among member systems.
